Occuscreen, LLC provides resources for prospective applicants and employees to help them understand their rights regarding screening. Below are submission forms to request a copy of your consumer report or dispute any content included in your report.

HOW TO REQUEST A COPY OF YOUR CONSUMER REPORT

If you are the subject of a consumer report prepared by Occuscreen, LLC and would like to request a free copy of your report, call Occuscreen, LLC at 888-833-5304.

HOW TO INITIATE A DISPUTE IN YOUR CONSUMER REPORT

If you are the subject of a consumer report prepared by Occuscreen, LLC and find incorrect or incomplete information in your report, you have the right to dispute this information (per the Fair Credit Reporting Act). To start a dispute you can either:

1. Call in to Occuscreen, LLC at 888-833-5304 and ask for a Dispute Resolution Representative.
2. Fill out and submit the form below:
