HOW TO INITIATE A DISPUTE IN YOUR CONSUMER REPORT
If you are the subject of a consumer report prepared by Occuscreen, LLC and find incorrect or incomplete information in your report, you have the right to dispute this information (per the Fair Credit Reporting Act). To start a dispute you can either:
- Call in to Occuscreen, LLC at 888-833-5304 and
ask for a Dispute Resolution Representative. - Fill out and submit the form below:
